We are recruiting Adult Support Coordinators on a fixed term basis of 1 year, who will endorse and act in accordance with the principles of personalisation, ensuring that care and support are person-centred and, as far as possible to put the people with whom they work in control of their lives.

Supporting people with the right care and information at the right time can help prevent future need, enable independence and promote positive wellbeing that will empower people to live the lives they want to.

About You

You will have the ability to undertake innovative assessments, support planning and delivering services and reviewing outcomes with service users, carers and their personal support networks. Addressing adversity and promoting service users independence and autonomy. Making sure the care and support you provide is personalised and, where possible, gives service users’ control of their lives. You will require the skills to work in partnership with health and voluntary organisations to achieve the best outcome for service users.

You will be have experience working in Adult Social Care, with an understanding of safeguarding responsibilities.

You will be able to use your skills and knowledge to make independent decisions and act as a lead when liaising with multi-agencies.

Experience of working with adults with care and support needs would be useful as well as knowledge of the 2014 Care Act.

You will be motivated, reliable, dependable, self-confident and able to work autonomously and above all, committed to represent Reablement and be part of a flagship for social care innovation where your flair for finding creative solutions can thrive.

We are recruiting to support our Assessment Team and Adult Early Help team and although these posts are primarily based within our St Ives and Ely areas, you will be required to travel across the county to support our satellite offices and will need access to a vehicle.

The role of Adult Support Coordinator is offered on a full time basis of 37 hours a week under Scale SO1 (£27,741 - £29,577).

IMPORTANT: This role will require you to enter the premises of a CQC-registered care home as part of your employment. Regulations which come into force from 11th November will require employees over 18 who enter CQC-registered care homes in England to have completed a full course of authorised COVID-19 vaccine doses at least 2 weeks before 11th November, unless they have a medical exemption. As part of your pre-employment checks you will need to provide evidence of your vaccinations or medical exemption. Your employment will not commence until this evidence is received.
