Position: Adult Transition Worker

Pay Scale: Grade J

This is an exciting opportunity for an Adult Transition Worker post. The successful candidate will be working with the young people of North Northants to support them if they need assistance from Adult Social Care once they turn 18 years old. As part of the role you will be working very closely with the schools in the area to highlight people who may be in need of Social Care assistance, you will also be providing advice and support to young people and their circle of support. This advice and support could be about what is available in the local community, what to expect once they turn 18 or just general advice on how the processes work within the Council.

You will work very closely with colleagues in Northamptonshire Childrens Trust (NCT), Health and in West Northants Council, both adults and childrens. Networking will be an integral part of the role and interpersonal skills are necessary. This position is suitable for someone highly motivated and confident in their skills and abilities. You will be supported by a Transitions Manager who will have oversight of the Strategic vision. You will also be working closely with our Learning Disability Team and Inclusion Teams. You will be working with young people who have various disabilities including, Learning Disabilities, Autism, Physical Disabilities and Mental Health needs. You will work in a strengths based way and be advocating for progression for people, to enable people to as independent as they can be and less reliant on formal services.

Part of the role will also be about collating data and ensuring that no young people get “lost” in the system.
