Are you passionate about supporting the needs of older people in the community? If so, we have an opportunity for you to join our Independent Living Service as an Independent Living Facilitator.

You will utilise your excellent communication skills to support older people in Cambridge by completing assessments, reviewing and redrafting support plans to ensure that residents maintain their independence and quality of life. Actively listening and sensitively responding to resident’s concerns is a key skill within this role and we will look to you to refer to other professionals such as social workers, OT’s, community nurses, GP where required.

You will build and maintain a number of relationships with partner agencies and contractors to ensure that all records are up to date and that any special requirements are communicated. You will monitor the environment in our sheltered housing schemes, reporting communal repairs, cleaning and other requirements to partner agencies to ensure that they are safe and well-maintained for our residents. We will also look to you to develop strong community links and partnerships to offer a choice of social activities and entertainments that will help maintain independence.

With experience of working with adults at risk and knowledge of health and social care needs of older people, you will have the ability to build and maintain working relationships with a range of different people to support the needs of the older people in the community. You will be self-motivated and well organised with the ability to work as part of a team and on your own initiative. Above all, you should be sensitive to the needs of others with an ability to solve problems which will improve the quality of life of our residents.

The working hours for this post are 08:30 – 16:30, Monday to Friday.
