The new Reablement North team are looking to recruit 5 office based RSW’s who are enthusiastic and able to think creatively, supporting frail or vulnerable adults with their individual Reablement journeys to enable them to regain their independence. Where this is not possible to undertake care Act assessments against eligibility to access ongoing community care packages. Good communication is essential to support the Supervisors within the new Reablement North team

As a Reablement Support Worker, your responsibilities will include:
• Respond to emergency situations in the community and support hospital discharges in partnership with health services, assessing who can be supported at home and providing short term support to maintain the person’s safety.
• Undertake risk, manual handling and medication assessments in order to safeguard both staff and person.
• Maintain case records. Including written, verbal and using our bespoke IT systems.
• Liaise with health care professionals. Maintaining good links with local communities and other agencies.
• Assess for, request and install low and mid-level equipment
• Undertake Care Act assessments for ongoing care

Requirements:
• A full driving license and access to your own vehicle. To be flexible with your working hours. A mixture of early shifts, late shifts and alternate weekends.
• A good understanding of Health & Social care through paid work, volunteering or personal experience.
