Continuing Healthcare Team Manager

Northamptonshire Adult Social Services (NASS), Continuing Healthcare (CHC) Team

Permanent – Full Time (37 hours per week)

The Northamptonshire Adult Social Services (NASS), The CHC team is a small team who are the central point of access for all CHC activity in to the Local Authority. We are seeking to appoint a full time Team Manager to oversee all activity within this team.

You will be managing a group of staff who are undertaking Continuing Healthcare assessments and there may be times when you will need to also undertake these assessments. You will be working with a wide range of customers from Younger Adults with Physical Disabilities, Learning Disabilities or Brain Injury and Older people with Physical needs or Mental Health issues.

You will have a strong commitment to ensuring the National Framework for Continuing Healthcare is applied correctly and robustly. You will work closely with colleagues in health and also attend Continuing Healthcare Panels. You will also be committed to assisting and empowering vulnerable adults to achieve their chosen outcomes.

For people who have Social Care Needs you will ensure that all assessments are Care Act compliant and strengths based. You will ensure that all Support Plans are focussed on Outcomes and that care is sourced to meet the need of the individual.

As a Countywide team you will ensure that all staff are engaged in the team and that there are systems in place to ensure their Health and Safety.

We are looking for a highly motivated, organised and creative individual with proven leadership and management skills. You will have experience of performance management and ability to motivate others. The role entails partnership working across a range of teams/services and an ability to be able to able to work to defined deadlines. You will have a proven experience of effectively supporting and managing staff and operational activities including budget and financial management.
