This is an exciting opportunity for a Strategic Transitions Manager. This is a real opportunity to influence the way we work for the young people of North Northants, developing and enhancing key partnerships and pathways to ensure a smooth transition into adulthood for young people with care and support needs. You will be a critical analyser and able to dive deeply into issues and be part of creative and outcome focused solutions to achieve better outcomes for people. You will be working closely with senior management to evaluate our current processes and you will make suggestions and work with teams to strengthen practice and performance. You will work very closely with colleagues in Northamptonshire Childrens Trust (NCT), Health and in West Northants Council, both adults and childrens. Networking will be an integral part of the role and interpersonal skills are necessary. This position is suitable for someone highly motivated and confident in their skills and abilities to influence the teams and our Adult Social Care senior leadership team. As part of the role, you will be determining the strategic direction for the transition service and ensuring that there are plans in place to move us forward. You may be delivering training to colleagues on certain key areas or attending meetings to work on new pathways and ensuring engagement from all concerned. You will also be line managing 2 transitions coordinators who will be working closely with the schools in the area to provide advice and support to young people and their families and ensuring that referrals are made into Adult Social Care in a timely way.

Part of the role will also be about ensuring that we know who is likely to require support from Adult Social Care within the current year and beyond so being able to collate this data and share key information to senior managers is essential. You will also be responsible for working closely with NCT to ensure that appropriate provision is available for when a person turns 18.
