Northamptonshire Adult Social Care Services has recently undergone a major Transformation Programme. We currently have a vacancy for an experienced Social Worker within the Complex and Specialist Older Persons Team - South. To undertake the more complex assessments, packages of care and long term support for people who are eligible for services that require a professional/specialist input. To identify unmet needs, safeguard, identify, promote, develop and challenge appropriate services and resources in order to meet the needs of customers and contribute to the development of service.

Working within a strength based ethos, you will be providing a range of social care interventions to adults with complex and longer term conditions.

You will be responsible for managing a caseload of individuals; assessing social care needs; developing, coordinating and reviewing care support, as well as ensuring that we meet our duty to safeguard vulnerable individuals. You will be committed to working with customers in a person centred way to support them to achieve effective outcomes.

You need to have good organisational skills, and will be an effective communicator used to working in conjunction with a range of other organisations, including health and other professional colleagues. You will also have a strong demonstrable commitment to working from a strengths based perspective.
