Social Worker – Adults & Older People

We have a full time (37 hours) opportunity for a Social Worker to work within the Physical Disability Team. This involves working with individuals between the ages of 18yrs – 75yrs with a range of physical disabilities and associated Informal Carers. We rely heavily on our Social Workers to deliver proactive, preventative and personalised care of the highest quality.

In this role, you will be expected to take the lead on a variety of cases, supported by a multi-disciplinary team to ensure that the care provided directly meets the individual’s needs. You’ll carry out assessments, identifying often complex needs and planning and implementing the required action.

We will rely on you to be vigilant in identifying safeguarding issues with a focus on supporting individuals to be protected from harm and abuse, whilst working within the principles of Making Safeguarding Personal.

As you’ll be working with adults, it will be crucial that you work in partnership with them in order to establish needs and ensure they’re met.

This isn’t just about delivering care, it’s about providing service users with choice and control of their lives and empowering independence.

About you

Needless to say, you’ll need to be a qualified and HCPC registered social worker to be considered for this role.

We’re also looking for someone who truly understands the complex needs of vulnerable adults, backed up by proven knowledge and experience.
